Some volunteer positions require official forms before service begins. Requirements vary by role, and not every volunteer will need every form.

You may be asked to complete forms such as:

  • Volunteer Agreement Form (DD Form 2793)
  • Volunteer Daily Time Record (DA Form 4713)
  • Volunteer Service Record (DA Form 4162)
  • Parental Permission Form (DA Form 5671)
  • Volunteer Safety Procedures

Your Volunteer Coordinator or supervisor will explain which forms apply to your position.
